Petition Sign-Out Sheet for Distribution Centers

This is a form to be used at each Petition Distribution Center. Have each volunteer receiving petitions provide their contact info and note how many petitions they are taking. If people stop in and don't take petitions but just want more info, please write INFO in that # of Petitions column. Thanks!
