Downloadable - Certificate of (Voter) Registration for Petition Circulators
Circulators must have this form completed by their Town Clerk to verify that they are registered to vote in Maine. Take it to the town hall where you are registered. The clerk will return it to you with a copy of your voter registration card (front and back) or some other proof of registration. Do this step now and send the paperwork into the campaign. It is not needed while you collect signatures, but it is needed when the petitions go to the Secretary of State's Office.
